Independent Person Directed Planning Services

PLAN Toronto provides comprehensive Independent Person Directed Planning services for individuals with disabilities and their families.

Person playing guitarOur Independent Person Directed Planning (IPDP) process consists of the following elements:

  • Private consultation to guide you and your family through a person directed planning process.
  • An independent, personalized planning session with highly experienced facilitators.
  • Formation of a personal support network, and ongoing facilitation of the circle to ensure the plan is effectively implemented over the long-term.
  • Ongoing contact with PLAN Toronto's planning facilitator.
  • A TYZE social networking site.
  • A copy of Safe and Secure: Six Steps to Creating a Good Life for People with Disabilities.
  • Registration in six workshops that explore the steps in Safe and Secure, as well as discounts on other workshops and events.
  • Group activities and opportunities to connect with other individuals and families in similar planning and network building processes.
  • Opportunities for follow up and support beyond the program.
